I was on the St. Lo at Samar and was one of the 434 picked up by the Dennis when the St. Lo went down.  I'll never forget how the crew of the Dennis gave us everything they had after we had lost everything.

The story of the plaque is as follows:  After the war, Mark Capalia, (a shipmate of mine on the St. Lo) became a harbor patrolman in Los Angeles harbor. One day while on patrol, Mark spotted a ship being towed to the scrapyard.  Recognizing that it was the Dennis, he contacted the scrapyard and advised them that this was the ship that picked him up when the St. Lo went down.  The scrapyard sawed the muzzle off one of the 5 inch guns, mounted it on a plaque and presented it to him.  Over the years, I tried to get Mark to donate it to the museum on the USS Midway in San Diego harbor--but he just couldn't give it up.  The St. Lo was originally commissioned as Midway and the name was changed just before we left Manus harbor for the Leyte invasion, so we only had the St. Lo name for two weeks.  The name was given to the present Midway (CV 41).
 
I'm attaching a photo of the plaque taken by Mark... he died in January.  I have been in touch with his daughter in hopes that something can be done with the plaque--I think it's priceless!!
 
The plaque is hard to read, but I have enlarged it and it says "This muzzle  end of a 5" 38 gun was removed from the USS Dennis especially for Mark Capalia who credits this ship with saving his life on October 25, 1944."

When he heard the explosions, Holly Crawforth ran to a forward ladder leading to the hanger deck. Someone cautioned him: ‘Don’t go up.’  “I started to turn back but I just had to see.”  Holly glimpsed smoke and fire sweeping across the deck and a huge hole in the flight deck just behind the after elevator. He ducked back below, and took a roundabout series of passageways back to Radio 2.

Radio 2 was dark, but the strikers were still there. “I told them to get the hell out. We went over to Radar, which had a door opening onto the catwalk.” The door was jammed but they quickly unhinged it, tumbled out and ran forward along the starboard catwalk, away from the smoke and fire. Holly inhaled the acrid scent of exploded ordnance. After emerging unscratched from the morning’s ordeal, it was hard to believe they’d finally been hit

Minutes before, Tom Van Brunt had nursed his TBM back to the St. Lo and found she was at last recovering aircraft. “I radioed our LSO: ‘Gabby, do you think you can bring me in on a right hand turn? I haven’t got any left rudder.’  He said he could as long as I had good control, but I had to wait until everybody else got aboard.”

Tom circled at 1500 feet and waited for the others to land. “All of a sudden the wing of a Japanese plane fluttered right in front of me. The plane had been shot down by flak.  I didn’t see the wing and almost hit it.” Then he saw a second plane careen towards St. Lo and watched as it too was shot out of the air.  What Tom didn’t see—and what St. Lo gunners missed as well—was a third Japanese plane approaching astern. “He came right up the wake, pulled up, nosed over and crashed into the flight deck. It was a huge explosion and you could see people right away jumping over the side.”

"It was just a real chaotic scene," said Holly Crawforth, who served aboard the ship as an electronic technician. "When he hit, all of a sudden there was this terrible explosion, we didn't really have much time. Our communication was out, but we could hear the bugle blowing . . . it was only 30 minutes until the ship was totally underwater."
